Become Pre-Qualified
At Phelps Construction Group, our process of subcontractor selection begins with a review of company information. In order to maintain our subcontractor/supplier database, we require that each subcontractor participate in an initial qualification process and provide annual updates. The link below will direct you to the pre-qualification questionnaire. Please complete all applicable sections and submit for review.
Before you get started, you will want to have the following documents in PDF form available for upload:
∙ Sample Certificate of Insurance showing current coverage and limits.
∙ Company Safety Policy
∙ A letter from your insurance indicating your Worker’s Compensation Experience Modification Rate (EMR) history for the last three (3) years.
∙ OSHA 300A reports for the last three (3) years.
∙ Completed FORM W-9 indicating current legal name and address.
